The important documents for participants to consult are

• the protocol, which describes the background, design and analysis of the trial;
• the frequently asked questions, which contain the basic guidelines for involvement with the trial, providing more detailed information than the protocol;
• the guidelines for filling in the Data Collection form;

Randomisation

Please remember that requests for randomisation of an investigator are to be sent to Mike Clarke at the UK Cochrane Centre rather than to the First Contact base in Cambridge, UK.

Randomisation requests can be made by

1. faxing the first page of the Data Collection form to Mike Clarke on +44 1865 516311
2. sending a copy of the first page of the Data Collection form by surface mail to Mike Clarke, UK Cochrane Centre, Summertown Pavilion, Middle Way, Oxford, OX2 7LG, UK
3. emailing mclarke@cochrane.co.uk using the following template, which you can copy and paste into your email. Please fill the seven pieces of missing information, deleting as applicable from the options separated by "//".
